Our Services
West Carleton Family Health Team offers a type 2 diabetes program that is focused on providing comprehensive diabetes care and empowering our patients to effectively self-manage their diabetes. Along with the primary care provider, our team of diabetes health professionals includes a pharmacist, registered dietitian and registered nurses who work closely with patients to help them make healthy choices to prevent or help manage their diabetes.
