Our Services
West Carleton Family Health Team offers a Type 2 Diabetes program that is focused on providing comprehensive diabetes care and empowering our patients to effectively self-manage their diabetes. Along with the primary care provider, our team of diabetes health professionals includes a pharmacist, registered dietitian and registered nurses who work closely with patients to help them make healthy choices to prevent or help manage their diabetes.

Group Sessions
We are running recurring virtual group sessions on lifestyle management for diabetes and prediabetes. Check out our Diabetes Group webpage for dates, sign up and more information.
