Current Status
Our family health practice has been serving Carp and surrounding communities since 1973. What started as a small practice with two physicians has expanded to twelve physicians on staff. In the spring of 2006 with funding from the Ontario Ministry of Health and Long Term Care, we became a Family Health Team. By employing allied health professionals, we are better able to serve the needs of the community, take on more patients who do not have a physician, and coordinate to provide the highest possible quality of care to our patients.

Family Health Team Model of Care

A Family Health Team is a patient-centered approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care in providing health promotion, disease prevention, management of chronic conditions and primary health care services. Along with your family doctor, our team of Allied Health Professionals work together to provide you the best care, when you need it. Our team includes the following Allied Health Professionals:
  • Nurse Practitioners
  • Physician Assistants
  • Registered Nurses
  • Dietitian
  • Pharmacist
  • Social Workers
  • Psychologists
  • Psychiatrist​

How does it work?

  • Your family doctor will continue to see you during regular office hours.
  • You will have access to other health care professionals within the Family Health Team, such as nurse practitioners, nurses, a dietitian, a pharmacist and a social worker.
  • You will have access to a Family Health Team doctor during extended evening and weekend hours for urgent problems.
  • If you see another doctor within our group for treatment, your own doctor will receive documentation about the nature of your problem so that he or she will always be up-to-date about your medical condition.
  • If you need assistance after-hours, you will be able to call Health Connect Ontario where a registered nurse will provide advice about your urgent health care concerns. With your permission, your doctor will receive a written summary of your call the next day, so that your doctor is informed of the nature of the problem and the advice you received.

Scribe Technology

Beginning in Fall 2024, some clinicians at the WCFHT use Ambient Scribe Technology to support more efficient notetaking in appointments. This technology is designed to streamline patient visits, reduce administrative burden, and improve overall care quality.

Waiting Room Art

The art you find in our waiting room and hallways is supplied by the members of the West Carleton Arts Society. These may be purchased by contacting the artist directly or by contacting the West Carleton Arts Society. A portion of the purchase price is being donated by the artist to Carp Health Access. You are encouraged to view these works of art when you visit the Medical Office. The art is changed by the artists about every three months so we have new art being displayed throughout the year.
