Sylvia Chan, BScPhm, RPh Sylvia is a clinical Pharmacist at the West Carleton Family Health Team. She completed her Bachelors of Science in Pharmacy at University of Toronto and has been with the team since 2006. Previously, she has worked at the Queensway Carleton Hospital, Elisabeth Bruyere Residence Long-term care, NIHB Drug Exception Centre, and Bruyere Pharmacy, but Sylvia’s passion is with primary care. Sylvia’s interests include chronic disease management, deprescribing in the elderly and resolving drug-related problems.
