Group Programs

The West Carleton Family Health Team offers some group programs for our patients. These programs are all free of charge and are designed and run by our team of health professionals. 

Nutrition and Lifestyle

Healthy Living with Cholesterol - Currently paused for summer break. Fully virtual. 
This program is run by our Registered Dietitian and it provides information to patients on important heart health topics such as: understanding cholesterol blood work, risk factors for heart disease, and lifestyle management strategies such as diet, physical activity, and stress management.

​Healthy Living with Diabetes - Currently paused for summer break. Fully virtual.
​This program is run by our Registered Dietitian and it provides information to patients on blood sugar management for people with Prediabetes or Type 2 Diabetes. It covers diet information such as carbohydrates, the glycemic index, as well as exercise and foundational information about diabetes and medications.
