Within the Patient Communications section you have access to Patient Questionnaires that allow you to forward information to us. Some of these require you to have a login so that we can identify you. It is explained in more detail on the associated pages.
In the future we plan to add more to the Patient Communications section as areas of interest are identified.
If any of your demographic information has changed such as your email address or phone numbers (home, work, cell), please send an email to firstname.lastname@example.org to advise us.
DO NOT SEND US ANY EMAIL MESSAGES REGARDING ANY MEDICAL CONDITIONS, BOOKING OR RE-SCHEDULING OF MEDICAL APPOINTMENTS. We will only accept emails pertaining to changes of address, phone numbers or email addresses. Our emails will only be read when our staff has available time.
