Patient Communication
The physicians and allied health professionals of the West Carleton Family Health Team are launching a new program to improve our communications with you.
If you have already registered with us, you may proceed to the questionnaire page using the side menu.
The program has 2 parts:
[1] For those patients who have provided their email addresses, we will be able to give timely notification of recommended health-related actions.
These may include:
- appointment reminders;
- reminders to have blood work and other medical tests when they are due;
- instructions about how to prepare for scheduled tests or procedures;
- notification of upcoming health-related events (like the flu-shot clinic each fall);
- general Health alerts or warnings;
- requests to complete on-line health-screening questionnaires.
Please note: sensitive medical information will NOT be sent by Email.
[2] Those patients who register on our website for a Health Questionnaire ID will have the opportunity to provide periodic updates of their health status by way of an on-line questionnaire tailored to their particular condition(s).
Your assistance in keeping your medical chart up-to date will enable better planning and coordination of your health care.
In some cases, the completion of a health Questionnaire and provision of self-monitored items like blood pressure, waist circumference or blood sugar (to name a few examples) may alleviate the need for an office visit.
Who is eligible?
Registered patients of the WCFHT who are at least 16 years of age.
How to participate:
If we already have your current Email address on file, please read the remaining material on this page, then proceed directly to the Registration page.

Note: if you are responding by email , please include your date of birth and first and last names as they appear on your health card.
For privacy and technical reasons, each patient must have a unique Email address. If you share an address with family or partner, to participate in this program, you will need to create a new address with your current ISP (internet service provider) or with a web-based service like Google's "gmail".
Once we have received your unique email address and recorded it in the system, an email will be sent to you within 3 business days with a link to the Registration page of our website where you can obtain your health questionnaire ID (username and password).
After obtaining your ID, you will be asked to login and complete a short on-line health questionnaire.
The results of this questionnaire will be reviewed by your physician or nurse practitioner and appended to your medical chart.
In the future, you will be asked to complete other questionnaires to maintain current information in your chart.

IMPORTANT
The WCFHT cannot accept general responses or questions from patients by E-mail. You should only respond by completing an on-line health questionnaire when requested to do so , or by phoning the WCFHT directly.
