If you are looking for a Family Physician, you should register with Health Care Connect, on-line or by calling them at 1-800-445-1822.
The West Carleton Family Health Team is accepting new patients from the West Carleton area. We work exclusively with Health Care Connect. You may advise HCC that WCFHT is your Clinic of choice.
**Please do not call our office as we will refer you to HCC. **
Q - I’m pregnant and need a physician for prenatal care.
A – Our obstetrical team of physicians will accept patients with or without a family physician during your pregnancy. If you have a family physician, you will return to their care after the birth of your child. If you do not have a family physician, you can make a request with the obstetrical physician, to accept you and your child into the practice after your delivery. If you are pregnant, call our office medical reception (613-839-3271) and request an obstetrical appointment.
Q – I want my family to transfer their care to WCFHT.
A – If you are a registered patient of WCFHT, you may request from your family physician the registration of your immediate family member (spouse/partner and children) to the new patient wait list. We offer this service to maximize the continuity of care to the family group. This is not automatic and depends on a number of factors: E.g. if your spouse has a family physician and has current medical needs, staying with the current physician may be in their best interest for continuity of care. Please wait until your next regular appointment, we will not accept appointment requests for this purpose only.
Q – I have a family physician that is too far away.
A – We are not accepting patients that have a practicing family physician. You should register with Health Care Connect.
Q – I do not have a family physician.
A – You need to register with Health Care Connect, on-line or by calling them at 1-800-445-1822. Health Care Connect (HCC) staff will find you a family physician that is taking patients in your area.
